Nursing Services

Our transitional care team bridges the gap from hospital to home after an injury or illness.

We provide a variety of clinical specialties including ventilator and tracheotomy care, wound care, IV antibiotics, ostomy care, central lines, and tube feedings. We also provide medical transportation to area appointments, which is included in the cost of your stay.

Our facility has a dedicated wound care certified nurse on staff, who assists the clinical team in putting together a treatment plan for complex wounds. We also have a wound certified nurse practitioner that rounds in the facility and works closely with the clinical team to monitor and improve resident conditions.

Assessment

In our facility, each resident receives a thorough assessment from a licensed nurse of their physical, emotional, and functional status. Additional assessments are performed as needed, based on caregiver recommendations, or if a change in condition is perceived. These assessments help us make sure our residents receive appropriate care and are as happy and healthy as possible.

Discreet support is available 24 hours a day from our professional staff.

We offer a broad range of services, including:

  • Personal Care – assistance with activities of daily living, including bathing, grooming, dressing, and medication management
  • Incontinence Management – daytime and overnight assistance
