To request medical records for a resident of one of our communities, please complete the Authorization for the Release of Medical Information form. Ensure it is signed by the resident or a legally authorized representative and include any supporting documentation, when appropriate. Email the completed materials to [email protected].
Please note, charges may apply for the retrieval, production, and transmission of medical records. Any charges will be assessed in accordance with the payment schedule set out in Minnesota Statutes § 144.292, subd. 6 and other applicable state and federal law.
