Meet The Team

The staff at our nursing care facility is dedicated to providing exceptional resident care.

Talia Pletcher, LNHA

Talia earned her bachelor’s degree at the University of Wisconsin-Eau Claire’s Health Care Administration program. She completed her skilled nursing facility practicum in 2010 and started her career in Senior Housing and Assisted Living. She has experience working for both for-profit and non-profit organizations and has managed the full continuum of care, including community home health care. She has served on several committees and professional associations within the industry. Talia has worked for Monarch Healthcare Management since 2018.

Rizza Malong, RN
Director of Nursing

Rizza graduated in 1994 with her bachelor’s degree in Nursing while living in the Philippines. She has experience working in the nursing field in various parts of the world before migrating to the United States in 2003. While in the US, she has worked in various settings in healthcare and in various capacities including 10+ years in the Skilled Nursing Facility and Assisted Living Facility settings. Rizza has also recently worked in the acute care setting prior to joining Monarch Healthcare Management in 2018.

Nathan Klaphake, LSW
Director of Social Services

Nathan earned his bachelor’s degree in Social Work and a Bachelor’s Degree in Sociology with a Minor in Philosophy from Winona State University. Nathan has worked as a social worker in community mental health, providing support to adults with severe and persisting mental illness which included social support groups, psychoeducation, and housing support. Nathan has also served as an acute care social worker with Park Nicollet Methodist Hospital prior to starting with Monarch Healthcare Management in 2018. Nathan is currently providing ongoing licensure supervision for newly licensed social workers entering the field. He is also pursuing his master’s degree in Social Work with Winona State University.
