Moving into a new living situation can be a stressful and difficult time for a loved one and their family. The staff at Crystal Seasons has special experience to get you and your loved one through the difficult start. We realize you may be feeling overwhelmed right now, so we have put together a few helpful mentions to prepare you for your stay.
We will greet and accompany you to your apartment and help you settle in. After a member of our team shows you the features of your apartment, we will complete an initial assessment to ensure you get the care you need when you arrive. You can expect to meet with a representative from each department within 48 hours to learn more about you and your preferences. Behind the scenes, these representatives will be working together as your interdisciplinary care team. They are the driving force in developing the plan of care for all our residents. We are committed to providing you personalized care in your new home.
We know that each individual is just that- an individual and our services reflect that philosophy. Crystal Seasons tenants can select from a board range of care options depending on their personal preference and needs during their stay with us. A customized care plan is developed to meet each person’s physical, emotional, and spiritual needs. This plan is periodically assessed by the individual, their family and our staff to allow for the most appropriate care at all times.
Our goal is to make your transition into our facility as seamless as possible. We know the decision to move into our facility is not one that is taken lightly. We will do everything within our power to help new tenants get acclimated to our community as quickly as possible.
