Here is what to expect at the beginning of our journey together.
Moving into a new living situation can be a stressful and difficult time for a loved one and their family, a skilled nursing facility, even more so. First, take a deep breath. The staff at Hillcrest Rehabilitation Center has the experience to get you and your loved one through this transition.
When You Arrive
After arriving at Hillcrest Rehabilitation Center and getting settled in, you can expect to meet with a representative from each department to learn about your preferences, usually within the first 48 hours. Behind the scenes, these representatives will be working together as your interdisciplinary care team. They are the driving force in developing the plan of care for all our residents.
After You Arrive
Hillcrest Rehabilitation Center will provide your medication, medical/hygiene supplies, activities, and meals. The only thing you will need to bring is clothing, assistive devices that you may use, and any personal items you would like to make your room feel more home-like. If there is ever anything that we can do to make your experience better, please don’t hesitate to let us know.
Our Goal
Our goal is to make your transition into our facility as seamless as possible. We know that the decision to move into our facility is not one that is taken lightly. We will do everything within our power to help residents get acclimated to their new environment as quickly as possible. If you or your loved one is worried about moving in, please let us know and we will work together to come up with a move-in plan that relieves any concerns.
