What to Expect
At Laurels Peak Rehabilitation Center, we seek to provide you with a seamless transition into our skilled care facility.
We’re committed to assisting you in making Laurels Peak Rehabilitation Center your new home with a seamless transition to our continuum of care campus.
Moving into a new living situation can be stressful and difficult time for a loved one and their family. The staff at Laurels Peak Rehabilitation Center has special experience to get you and your loved one through the transition. We realize you may be feeling overwhelmed right now, so we have put together a few helpful hints to prepare you for your stay.
When You Arrive
When you arrive at our facility, we will accompany you to your room, and help you settle in. After a member of our team shows you the amenities in the room, we will complete an initial assessment to ensure you get the care you need. You can expect to meet with a representative from each department to learn about your preferences, usually within the first 48 hours. Behind the scenes, these representatives will be working together as your interdisciplinary care team. They are the driving force in developing the plan of care for all of our residents. We are committed in providing you personalized care in your new home.
What to Bring
Make sure you pack several loose, comfortable outfits to wear if you will be participating in physical and occupational therapy. These outfits should be easy to get on and off because you don’t want to potentially aggravate any injury you may have. Sweatpants, shorts, t-shirts, and sweatshirts are good choices. Additionally, you will need to bring pajamas, undergarments, and any toiletry items you may need during your stay, as well as assistive devices like glasses, hearing aids, or equipment you were given at the hospital (braces, walker, etc.). If you forgot something, don’t worry. Simply let the staff know and they will accommodate you.
Our Goal
Our goal is to make your transition into our facility as seamless as possible. We know the decision to move into our facility is not one that is taken lightly. We will do everything within our power to help new residents get acclimated to their new environment as quickly as possible. If you or your loved one are worried about moving in, please let us know and we will work together to come up with a successful move-in plan that eases any concerns.
